The application form should be accompanied by a forwarding letter from the head of the institution where the candidate studied. The hall-ticket of the examination should also be sent.

After three months (of joining the course) in addition to the fee, a fine of ₹ 25/- also should be remitted.

This can be obtained by applying on the prescribed form (No 19) to 'the Controller of Examinations, University of Kerala, Thiruvananthapuram-34' with a fee of ₹ 1000/- with ₹ 100/- as fine within 5 years, ₹ 400/- as fine after 5 years and an affidavit from the Notary Public on ₹ 100/- stamp paper, stating that the certificate had been lost irrecoverably and in case if the original certificate happens to be recovered susequent to the issue of the duplicate, the duplicate certificate will be returned to the University for cancellation and the course certificate from the Principal of the institution where the candidate had studied for that degree course.

In case of Private Candidates, instead of Course Certificate, an Identification Certificate from a Gazetted Officer is to be submitted.
